Join Our Team as a Pre-Transplant Coordinator!

As a Pre-Transplant Coordinator , you will play a vital role in the transplant journey, guiding patients through every step of the process—from referral to evaluation, and beyond. Your Responsibilities

In this dynamic role, you will:

• Communicate Effectively: Use therapeutic communication to engage with patients, families, and the transplant team, ensuring everyone is informed and supported.
• Assess Needs: Identify and respond to the clinical, psychosocial, and economic needs of transplant patients and their families.
• Coordinate Evaluations: Work with an interprofessional team to facilitate comprehensive evaluations for potential transplant candidates.
• Educate Patients: Establish educational goals with patients and families, providing them with essential information about the transplant process.
• Monitor Progress: Keep track of patient status and ensure timely communication regarding listings, denials, or postponements.
• Participate in Events: Engage in transplant events and contribute to departmental goals for patient satisfaction.

What We’re Looking For

To thrive in this role, you should have:

• Education: A Bachelor’s degree or higher from an accredited School of Nursing.
• Experience: Four years of nursing experience or three years of Oncology/BMT-specific experience. Previous transplant acute care and/or ICU experience is preferred.
• Licenses: RN - Registered Nurse - Texas State Licensure (must obtain permanent Texas license within 60 days if establishing Texas residency) and BLS - Basic Life Support (AHA).

• Strong communication skills to effectively interact with patients and healthcare teams.
• Excellent organizational and problem-solving abilities.
• Knowledge of OPTN/UNOS/NMDP/CIBMTR/FACT policies and listing requirements.
• Proficiency in Microsoft products.
